It is my understanding that come pickup time for the father, the mother can
simply say "the child's sick, thus they won't be able to go with you" and
refuse to hand the child over to the father. That said, does the father have
an equal right to do the same to the mother when it comes time to hand the
child over to her?

That is an all too common excuse for visitation violators.  First off,
I'd still demand to see the child.  If the mother refuses, I'd drag her
into court and make her produce the doctor's note.  Also, you can kill
off this situation before it occurs by inserting a clause in the custody
arrangement that requires a doctor's notice before visitation is denied
because of health.
And no, two wrongs don't make a right, even though three rights make a left.
